 How can I be an artist at heart, someone who celebrates the human form, and be in the business of changing it? I thought for a long time that a plastic surgeon’s job was to make you look better. A plastic surgeon’s job is to make you feel better. The only way I can make you feel better is by making you look like you think you look. My first year at Georgetown, an NIH psychologist presented a study on plastic [surgery]. She found that people with the most confidence are those whose body image, what they see in the mirror, is very close to their self-image — what they see when they close their eyes at night. If I can achieve an approximation of these two things, then I’m going to make you feel better, and making you look better is secondary. Women want to control their own destiny — that’s all. The last thing they want is another man telling them how to look. 


This obsession with looking good? Plastic surgery is a follower, not a leader in this. Women’s need to look better is driven more by celebrities, the media and cameras in cellphones. Every day, I see young women taking photos of themselves, looking at it and shaking their heads. They don’t need Botox, they need a better camera. My next career will be a plastic surgeon to pixels. 

At least once a week someone says, “You probably look at people and always see their faults.” Not true. I look and think: If they were a painting, what could I do to bring out their beauty, make them look a little better? I’m luckier than Michelangelo. When he was chiseling away at David, he made his bicep a little bigger and his nose a little smaller. I bet Michelangelo would have loved to make the model, not the stone, look better. 

In Waldorf, where my first practice opened, [a lot of] women [are] walking the streets with my breasts in them. I pretend I don’t know them. Sometimes they call and say, “We saw Dr. Hakki at Captain Billy’s. Why is he so stuck up when he’s so nice in the office?” I don’t want to embarrass them. If someone passes you on the street and can see my breast augmentation, my Juvederm, my butt lift, I have failed. Your own mother should see you and have no idea. 

I say no every day. When a woman says, “My husband likes big boobs,” or “I want to save my marriage,” I will not do it. If you are changing who you are for someone else, I will not help. When a patient says she woke up with a bad feeling in her gut, I’ll cancel the surgery. I’m still superstitious; the artist always controls the scientist.

From the tight-faced women of The Real Housewives of Orange County to the enhanced

bosoms of the Jersey Shore gals, women undergoing plastic surgery is more socially rampant

than ever. But what about men? While a handful of DC women, such as Fox News personality

Greta Van Susteren, are happy to discuss the various nips and tucks that keep them looking

fresh, most men tend to be mute on the topic. Of course, that doesn’t mean they aren’t undergoing

procedures all the same. Dr. Ayman Hakki, the founder and CEO of

Luxxery, a plastic-surgery office in Maryland (3010 Crain Hwy., Ste. 400, Waldorf, 301-843-

9769; luxxery.com) with a walk-in Botox boutique in Georgetown (2141 Wisconsin Ave. NW, 202-

333-9294), notes he has seen a recent increase in men seeking consultation for cosmetic surgery.

“Men go for things that make you look different without making you look too different,” Hakki

explains. The two most common procedures: eye lifts, and liposuction on their love handles.

“Men wanting to fix their ‘tired’ eyes are the biggest portion of new male clients these days,” says Chevy Chase–based plastic surgeon Dr.

Wendell Gordon Miles. “The biggest uptick I have noticed is in men moving up in their careers.

The thought of interviewing against younger competitors seems to motivate them to come in

for consultations.” The men vary in age, but, as Miles notes, they tend to be in their mid-forties

and -fifties. Hakki describes two very distinct groups who come into his office: “super jocks”

and “executive types.” The former are military men who need to pass very stringent physical

requirements to continue moving forward in their careers. “Their neck and waist measurements

need to be certain ratios or they can’t fly,” he says. “We’re talking about men who are in

great shape but still have love handles.” Alternately, the executive types get eye lifts or

facial Botox in an effort to look younger when competing for jobs. “They feel threatened by the

young bucks coming up the ranks,” Hakki says. Still, the surgeons caution that cosmetic surgery

is not a decision to be taken lightly. There are lasting  effects on both a patient’s appearance and his wallet.

Procedures can alter one’s looks for years to come, and most cost between $5,000 and $10,000—

and are not covered by health insurance. Before operating, Hakki sits down with his patients to discuss

their decision to get surgery, and occasionally, he spots red flags. For instance, he does not operate

 on men who have unrealistic expectations of how an eye lift may change their lives. “The man who

tells me he is doing it because he thinks he is going to be 18 again, or because his wife is leaving and he

wants to win her back, those are the men I turn away gently,” he says. But not all men are going under the

knife to recapture their youth. For executives or members of Congress who are constantly in the

public eye, getting cosmetic surgery can be a simple public relations decision to improve their looks on

camera—that is, as long as it is done right. One local public relations maven recounts the story of a client,

a powerful executive, who had a botched eye lift— an unfortunate incident she described as unnatural

in its complete symmetry. The PR person had to advise on handling the fallout, as the surgery could

obviously not be undone. “He wore contacts, so I advised him to wear glasses until it settled in

because the lines of the glasses block the eyelids,” she remembers. Another option is to stick to non-invasive cosmetic procedures, where

 

local physician Dr. Philip Schoenfeld, founder and medical director of Renu Med Spa, has seen a spike in male clients. “These days, men are

entirely more comfortable with the concept,” says Schoenfeld, who adds that most come to his office with their wives and speak freely about

the procedures. Schoenfeld is also seeing more men opt for laser hair removal, a relatively quick and painless way to change ones appearance—

and a treatment that is becoming increasingly common. “Neck, back, and shoulders are the most popular places. That’s like a manicure

these days,” says Schoenfeld of the in-and-out ease of the process. “Let’s face it, hairy backs are out.” But, in general, the advice from professionals

is to keep it simple. Man or woman, the idea is to be the best version of yourself—not a completely different self altogether. CF

"Dancing With the Stars" pals Bristol PalinKyle Massey and his brother Chris have signed on for 10 episodes of an untitled project produced by Associated Television International for the BIO Channel. The news, announced Monday, does not come as a surprise, especially after the recent speculation over Palin possibly sprucing up her appearance.

After Tripp's mama stepped out on the red carpet for the White House Correspondents' Association Dinner, many wondered if she has had some work done. While no reports have been confirmed as to whether she has had cosmetic surgery, celebrity plastic surgeon Dr.

Ayman Hakki of Luxxery, in Georgetown and Waldorf, seems to think she has.

"She looks like she had liposuction of the neck. I can tell because her cheeks are just as plump as they used to be, while her neck is much thinner. She might have also had fat transfer to her jaw-line and chin to give her more definition," Hakki explained.

While a chin implant is a possibility, it isn't necessary, he said, since you can now use your own fat to accomplish the same thing.

"She may have also had a subtle amount of fat put in her midface region because her nose looks smaller. Accentuation makes the center of your face fuller and the sides thinner," Hakki said.

We were unable to reach Palin for comment.

Buttock Augmentation: From Consultation to Recuperation

Lately,buttocks augmentation has become a serious trend. These days, I am finding that beauty is measured more with a lady’s booty than anything else. The media is preoccupied with voluptuous perky buttocks, A.K.A. “Booty Beauty.” I covered all the illegal injectable materials used as alternative approaches for butt augmentations in our February issue of Your Health “Beware: Butt Augmentation.”Allow me to address a new, professional butt lift method. A buttocks augmentation, or “Brazilian butt lift” is a surgical procedure that involves liposuction combined with fat transfer into the buttocks. When aggressive,regional liposuction is performed along with the fat transfer, the procedure becomes the safest and most reliable method for achieving good, measurable,long lasting results. The liposuction portion of the buttocks augmentation procedure is the crucial aspect of the procedure. First and foremost, it is performed using standard liposuction techniques and not ultrasound or laser-assisted.This will ensure the viability of the harvested fat. It is also generally performed on particular areas that compete for attention in the buttocks region. It can do wonders to enhance the appearance of the buttocks. The foundation of all Brazilian butt lifts is the dramatic re-establishment of central fullness.This is achieved by careful planning of areas of liposuction and areas ofaugmentation. The secret lies in liposuction of the flanks, but mostimportantly the gluteus V shelf, which I refer to as the “fanny pack.” Thisarea, the sacral area immediately above the buttock crease, is what sculpts anddefines the curve that extends from the top of the buttock. The extent ofliposuction and amount of fat placement varies from surgeon to surgeon. A greatresult in the Brazilian butt lift is the combination of optimal viableharvested adipose tissue placed near the upper half of the buttocks and thepatient’s preference area.  This will produce a visible ledge on thebuttocks with a soft texture. The procedure can produce dramatic results. Allit takes is a few simple steps.


Consultation:  Meet your doctor. You and your plastic surgeonwill be able to determine whether buttock augmentation is right for you.Patients often have a preferred area for their planned liposuction and have aparticular, ideal buttock shape in mind. While in a standing position, you andyour doctor will be able to look in the mirror. At that point you may decidewhat areas of liposuction you want and where you want fat to be placed for afully satisfying outcome.  To accomplish a smoother silhouette, yoursurgeon may still recommend additional areas of liposuction for optimalsculpturing results. You are not considered a candidate for a Brazilian buttlift if you have unrealistic expectations or a medical history that does notpermit you to have elective surgery.

Surgery:  On the morning of your surgery, you will be asked torefrain from eating or drinking as part of anesthesia requirements. Yoursurgeon will address any questions you might have and mark your operative sitefor the planned surgical procedure while you are in standing position. Youranesthesiologist will describe the anesthesia process and address any questionyou might have.  Your surgery may take 2-3 hours under anesthesia. Differentsurgeons and surgical facilities use different anesthesia methods, yet a BrazilianButtlift can be performed under local anesthesia with intravenoussedation, in which case the patient can breathe independently during the entireoperation. This is the preferred methodsince it is more comfortable. Once the surgery is completed, an abdominalbinder (like a corset) is placed to provide a slight compression to theliposuction area and the patient is transferred to a short stay in the recoveryroom.  Patients usually go home within an hour of recovery.

Recuperation: You will be discharged to a family member or a care-giverwith all your proper medication to begin your recuperation process. Yoursurgeon will give your care-giver your discharge instructions. Otherinstructions include absolutely no cigarette smoking. Smoking will prevent yourhealing and beautiful transformation. You are encouraged to walk to thebathroom and around your house, and a follow up postoperative visit will bescheduled for the next day. It is also recommended that the binder be worn fora minimum of two weeks after surgery.  An ideal recuperation period for abuttock augmentation is one week to ten days or an average of seven workingdays. Some patients can resume their work even earlier provided they do notexert physical activity during the healing period.  Finally, it isrecommended that you resume your normal physical activities gradually in orderto avoid any complications.
