Breast Size Matters

To women in their 20s, 30s, 40s and even 50s, breast size is no small issue.  First comes the decision to join the ranks of the “surgically endowed”, then comes the harder choice of getting the size right.


With the abundance of preconceived negative ideas about electing to have plastic surgery, to join or not to join the ranks of the surgically endowed becomes an issue of self-esteem and plain human sexuality.  With the number of women undergoing breast augmentation increasing in staggering percentages year after year, understanding the benefits that occur from undergoing breast augmentation has been the subject of many studies. Numerous studies have been conducted showing a direct correlation in the improvement of women’s self esteem and sexual satisfaction and an absolute psychological improvement following breast augmentation surgery. These studies show that patients seeking breast augmentation merit respect, compassion and understanding instead of dismissal and stereotyping.
Ideal patients are women who do not view plastic surgery and breast augmentation as the end-all cure to their sexual afflictions and self esteem.  An ideal plastic surgeon has the ability to screen patients with serious underlying psychological issues and elects not to operate on patients who feel that their life will completely change after plastic surgery.  The combination of the ideal patient with the ideal plastic surgeon leads to what the doctor ordered, “Just the right size breast” from the first time.


Augmented breast have a spherical shape with an outward projecting areola and nipple, a rounded contour, narrow base with a wider cleavage and fullness on the top of the breast.  Enhanced breast have a natural more youthful appearance when the size is right.  The one decision about breast augmentation that often goes wrong is size.  A size satisfied patient is a patient who feels confident about her ability to evaluate her own plastic surgeon’s ability and advice.  Factors such as the 34C ideal cup size stigma, ill advice from friends, the wrong surgeon or the wrong perception of the ideal final results can lead to dissatisfied patients. In my twenty years of consulting patients in the field of plastic surgery, and in a practice with over two thousand satisfied breast augmentation patients, I have been able to accumulate a few pearls in order to help you get it right the first time on  “breast size matters”:

    •    Let your body structure dictate the cup size and not some celebrity chosen implant size.  This is not a one size fits all matter.
    •    The implant size your best friend chose might not be the size you should blindly choose
    •    Talk with your surgeon about your goals and base your goals on your original breast size and shape
    •    Check out your surgeon’s own patients before and after photos
    •    Check out the nuances that separate your plastic surgeon from the next and make sure you like his philosophy towards your procedure, as well as his personal and professional profile
    •    Check out your surgeon’s artistic ability. Can he or she draw or illustrate your procedure and your options or rely on computer generated images.  A plastic surgeon with no artistic talent is like a painter with no brush.

Dr. Hakki & Ginuwine - Washington Post

What happened to the days when there was no relationship more sacred, no trust more precious, than that between a celebrity and a plastic surgeon?

Last Friday, R&B star Ginuwineand his wife, the rapper Solé, paid a "surprise" visit to the Waldorf offices of Ayman Hakki.... who promptly sent us a press release about it!

Turns out the visit was strictly social, we're told. The couple, who make their home in the singer's native Prince George's County, met the doctor at a Georgetown party a few months ago; they explained that they happened to be driving past his new Luxxery Medical Boutique, recognized the name and dropped in to say hi.

Which led to the most shameless press release of the month: "They were awed by the services that Dr. Hakki offers. Dr. Hakki gave [them] a personalized tour of Luxxery, while also explaining some of the most desirable treatments available today. 'Adipose Accentuation has taken the place of Breast Augmentation in regards to popularity,' says Dr. Hakki. 'It's the newest craze not only in this area, but also around the country. In 2001, when I was featured on MTV in a breast augmentation special, that was the most popular plastic surgery procedure. Nowadays, Gluteal Accentuation (Buttock Enhancement) uses adipose stem cells to round out the buttocks and has taken the top spot as the most popular procedure in my practice. It's a national craze!'." That was it. No one went under the knife.

So why are we telling you this? Mostly because, after hearing all about exactly where the doc could reinject your fat if you so choose, Ginuwine delivered a pretty excellent punch line:

"My wife's perfect," he said, "but thanks for sharing the info!"

That's according to the press release, but we reached Ginuwine, who laughed good-naturedly and confirmed he said it. Well played, sir. (We'll give him some equal-time PR now: He's promoting the new album, "A Man's Thoughts," and exploring a possible reality-TV project.)

Acne Scarring Management: How We Do It

   The key to management of patients with acne scarring begins with a thorough analysis and a clinical examination with an emphasis on determining the cause of scarring, preventing scarring, and the types of scars. Once all these factors have been determined, we can design the options for treatment of the scars.

 A scar, in the simplest terms, forms at the site of injured tissue. In the case of acne, the injury is due to the body’s inflammatory reaction to sebum, the oily substance produced by sebaceous glands as well as bacteria and dead cells in the clogged sebaceous follicles. The type of scarring that results in people who are susceptible to scarring ranges from depressed scars such as ice-pick scars, to raised scars such as keloids, to hyperpigmented scars and dark spots. Facial scars resulting from acne regardless of how they manifest themselves often give the illusion of facial asymmetry or poor skin texture. My goal in the treatment of facial acne scarring is managing by way of removal or lightening of such unsightly areas for my patients regardless of skin color or ethnicity. My objective is to give the skin a more acceptable physical appearance with the understanding that total restoration of the skin to the way it looked before the occurrence of acne is not possible. Our treatment approach will accomplish a tremendous improvement of the appearance of the skin.

Old and new methods in treatment of acne scarring are only effective when taking into consideration the patient’s skin type and ethnic background. Knowing the patient’s susceptibility is important in the choice of techniques for management and effective treatment of acne scars. A variety of treatments are presented to the patient ranging from old methods such as the age old hydroquinones and peels to new methods such as laser treatments and fat transfer or adipose accentuation of the sub-dermis. Our approach consists of a combination of old and new techniques integrating medical strength product regimens, peels, microdermabrasions, laser treatments as well as stem cell laden fat transfer.

The scar treatments currently availableinclude:

Peels Only deep peels such as phenol based peels work on acne scarring yet are not recommended for dark or asian skin types. Worth mentioning is the new Vi-Peel™, a medical grade peel specifically designed for darker skin types and works on all skin types, it offers significant improvement to dark spots, hyperpigmentation and acne scarring in African American, Latin and Asian patients.

Microdermabrasions A surface form of dermabrasion, microdermabrasion using crystals or crystal free passing through a vacuum tube to remove surface skin requires multiple treatments and may not have an impact on improving acne scarring. However, a microdermabrasion performed immediately prior to a Nd-Yag laser treatment such as the Cutera Genesis, contributes to a better laser procedure. A combination microdermabrasion-complexion blending laser treatment can help significantly to improve scars.

Laser Treatments Laser and light based treatments for acne scarring and hyperpigmentation have increasingly become the appropriate approach where other methods have failed or as an integral part of a combination treatment plan that combines traditional methods such as topicals, peel and systemic approaches. For example the results with the Nd-Yag Cutera Laser Genesis treatments preceded by a microdermabrasion produce an excellent result for patients with hyperpigmentation and old acne scarring. The treatment results in smoothing out skin texture and reducing visible scars and blemishes. Scar tissue can be recontoured and redness around healed scars and acne lesions can be reduced depending on the type of wavelength and intensity of the laser used.

Fat Transfer When used to correct deep contour ice-pick scars resulting from cystic acne, fat transfer where fat is taken from another body site and prepared for injection into one’s own skin can elevate depressed scars with the longest lasting results than all other methods. Recent studies have proved that the newly injected fat contributes to the healing of the surrounding tissue where it is placed due the high number of stem cells in the transferred fat. The stem cells are involved in the repair of the skin and a remarkable repair of scars as well as an enhancement of the quality of the skin underlying the transferred fat. Multiple fat transfer procedures can achieve more of a permanent result.

Products Patients seeking treatments of acne scars should also commit to a customized home care regimen that combine cleansers instead of traditional soap which can strip the skin of its natural oils, with topical retinoids (Retin-A) to reduce blackheads, whiteheads and induce skin cell turnover with the added benefit of reducing wrinkles, a skin lightening cream or hydroquinone to reduce or remove hyper pigmented scars, and has an increased bleaching quality when used in combination with the retinoid. No regimen is complete without a moisturizer and a sunblock or a combination moisturizer sunblock as the final step to any customized home care regimen. The decision to seek treatment for your acne scars depends on many factors, the most important one being the way you feel psychologically and emotionally about your scars and whether or not you are willing to live with your scars. Another important factor would be whether you have the budget and discipline to seek the treatment options presented since acne scar treatments is not covered by health insurance. Luxxery's Double X's

People often ask me why I chose the name Luxxery for mymedical boutique. The idea of Luxxery is simple. We offer an absolutelyluxurious experience. Furthermore, the slightly skewed double X’s in our logo capturesomething fundamental about my approach to making my clients happy. Theyrepresent the double image people have of themselves. The relationship betweena person’s self-image and their reflection or body image is what makes themwant to stand in front of my “Magic Mirror.” Plastic surgery is driven by this disconnectbetween what we see in our reflection and what we would like to see.

 

The FX Networks air a popular show called Nip Tuck about twoplastic surgeons who share a practice. They start episodes with a consultationthat begins with the question: “Tell me … what do you hate about yourself?”

Clearly this is a fictitious approach no professional wouldtake. Instead, I ask my patients the following:

“If you were standing in front of a magic mirror, and couldchange anything about yourself; what would it be?”

 

This starts the dialogue. It’s imperative that I conceptuallysee what she wants, not just hear it. If I cannot see it, I simply cannotdeliver it. Today, this “seeing” is more difficult than it may sound. Women spenda lot of time researching on-line exactly which procedure they think will fixtheir issue. No amount of research can replace a face-to-face consultation witha board certified plastic surgeon and a mirror.

 

I do not have a magic mirror and I make that clear, but I dohave magic markers and a pencil. Though it may sound strange, I often draw onthe patient, and through this process she can see on her own skin where I willbe making changes. I then draw the before and after and hand her a copy of mydrawing. At times, the process allows the patient a sense of optimism, knowingthat the marks and drawings were made by a plastic surgeon who’s an artist, andunderstands how to communicate with her in order to give her a great result.

 

As a man dedicated to the art of plastic surgery, I try to makesure that once I have worked on her, the image she sees in that mirror conformsto what she wants. Allow me to tell a fairy tale to illustrate how I feel I understandwhat women want.

 

Once upon a time, a young prince lost a war with aneighboring king and was condemned to die. Appearing in the King’s court theprince was then offered a deal. Tell me what do women want, and I’ll spare yourlife. The prince then asked; but how will you know if it’s the right answer? Theking turned to the women around him and said; I’ll know. The women, finding theprince so charming, all smiled as they nodded in unison.

 

The prince hurried off to ask everyone in his principality,and no one could answer the fatal question. He was told of a witch, who livedin the forest, who just may be able to give him the answer. He was warned thatshe’ll extract a heavy price. He rode his horse into the forest, and had todrag his spooked horse to the miserable witch’s hut. He tied up his horse andentered the cold damp and smelly hut through a creaky door and in a corner sat thewitch…warts and all. She pointed a gnarled long finger at him, and proclaimed;I know why you came. Startled he said: How? She said; I’m a witch I knoweverything and I can do anything I want to do. He asked her, and she looked at themagic mirror at her side and said; I’ll tell you, but you must marry me. He washesitant, concluded that it was better than death, and agreed. She then said;women want to control their own destiny.

He galloped off to the Kings court and before the clock struckmidnight he blurted out:

“Women want to be in control of their destiny!”

The women of the court all nodded, and his life was spared. Backat the hut, he was surprised to find a beautiful woman in front of her magicmirror. He was delighted. How did you do that? He asked.

“I’m a witch,” she responded. Then she added:

“I can look like this for you here in private in the hut, orin public, you must choose.”

He thought for a moment and said:

“No … you choose.”

She transformed herself into a beautiful princess inside andout side her hut.

 

Many chose to look the same way they do in front of themirror, but for the ones who would like to change a few things aboutthemselves, I offer educated, confidential and informed advice. The Art of Fillers

All fillers are either temporary (Restylane Juvederm are Hyaluronic acid based), semi temporary (Radiesse), or permanent (Fat, Artecoll and Dermalive). All these fillers (with the exception of fat); from the most absorbable (Collagene) to the least absorbable (Gortex) fall into the category of allo-dermic (not of one’s body) fills, and all carry side effects, see first reference bellow.

They are becoming popular because they are relatively simple to use, and the injection is easy to recovery from. Face lifts, peels, lasers and even fat-fills cost more, require higher expertise, and a longer recovery period. My solution is Face Accentuation and by using this sequential method I enhance each individual’s underlying facial youth by; first using injectable fillers, then a selective-mid-facial-filling with permanent fillers (like fat), then doing a surrounding-facial-thinning, and using skin tightening only when necessary.

Suction-lipectomy was invented by Teimourian in 1975. He later in 1985 started to inject the fatty product of suction into the face. Obagi pioneered the use of peels to tighten and rejuvenate skin. Coleman refined fat-filling and trademarked it as Lipostructure. Little invented a method of permanent central facial augmentation that requires extremely skilled surgical manipulation. He then coupled it with an Omega lift and coined it “Facial Sculpture”. Bitar in 2002 used many artificial types of filler (see references).

My approach borrows from these and other cosmetic advancements, but it differs from them all in that it makes the case for central augmentation and peripheral reduction, only using lifting when the first two methods (with or without peels) are insufficient to achieve desired skin tightening. This sequential, simple and quick procedure achieves the desired accentuation; bringing out-naturally-the features we used to love in our face.

To understand this new all embracing procedure one must realize the outcome of each individual procedure. Lifting makes the face look gaunt, suctioning makes the face look empty, and filling makes the face look puffy. Face Accentuation, on the other hand, simultaneously fills the center of the face, reduces its perimeters, and trims the excess skin if peeling is insufficient to do so. It is a totally new way to enhance, not detract from, the beauty of our face. Lifts (as a primary rejuvenation technique) is becoming unpopular and my Accentuation restores youthfulness without relying solely on artificial substances.

My method differs from others in that it is both natural, costs less, and is eventually permanent. Standard cosmetic interventions often give rise to a “plastic” look. Replacing short-lived treatments, permanent foreign substances, and the more radical surgical procedures, Face Accentuation combines modalities in a set-sequence that delivers a rejuvenation that is both natural and long-lasting.
